Helping People with Disability Safely Re-Enter the Community

At Livington Care Services, we work closely with Hospital Liaison Officers (HLOs) and NDIS hospital discharge planners and their families to support individuals with disability as they transition from hospital to community living—beginning their journey toward a safe, stable, and meaningful support in our SIL homes or their residence.

Our Rapid Hospital Discharge Program (RHDP) is designed to ensure that individuals who are clinically ready for discharge can exit hospital settings quickly, safely, and with dignity.

Our Specialist Support Coordinators work closely with participants to find and secure long-term housing that meets their individual needs and goals.

Working alongside allied health professionals and other service providers, we ensure each participant’s NDIS plan is aligned with their current and future support requirements.
